**Nurse Practitioner**

To provide independent advanced level nurse practitioner clinical expertise to patients and their families on an ongoing basis. This includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and specialty protocols (e.g., chemotherapy) as defined by standardized procedures or independently based on licensure. This may include non-invasive or invasive procedures, surgical procedures, including operative first assist per approved standardized procedures as identified by the list available on the Medical Staff Office. **Overview of the Role**

The Alignment Care Anywhere program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social support in the comfort of their own home. This program is offered to eligible patients at no cost to them with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible.
