**Job Description**

- Conduct comprehensive assessments, physical exams, and develop personalized care plans. Manage both acute and non-acute conditions and provide clear documentation.
- Work independently and as part of a multidisciplinary team. Participate in clinical rounds, meetings, and quality management programs.
- Act as a patient advocate, facilitating family conferences, and ensuring education on care plans.

**Requirements**

- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar. Board certification preferred; active state licensure required.
- Minimum 2 years in a clinical setting with leadership experience preferred. Proven ability to manage a patient panel effectively.
- Exceptional clinical skills, advanced communication, and problem-solving abilities. Experience in geriatric care is a plus.
