**Hospice RN Case Manager**

As a Hospice RN Case Manager, you will be providing individualized, coordinated hospice care to bring safety and comfort to the patients and families. The holistic approach encompasses physical, emotional, and spiritual well-being. You will have independence and flexibility in the field to visit patients one-on-one in their home environment.

**Job Description**

- Provide in-home hospice care to patients within a general territory in the Southern section of the Twin Cities Metro area.
- Participate in an on-call rotation as part of full-time days, generally Monday - Friday from 8:00 am - 5:00 pm.

**Requirements**

- Licensed to practice as a RN in the state of MN. License must be active and in good standing.
- Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate.
