- Evaluate, diagnose, and manage a variety of wounds (pressure ulcers, diabetic wounds, surgical wounds, venous/arterial ulcers)
- Perform procedures such as sharp debridement, tissue biopsies, NPWT, and dressing changes
- Create and adjust wound care treatment plans
- Educate patients, families, and facility staff on wound care and prevention
- Maintain complete, HIPAA-compliant documentation using secure EMR
- Drive between patient locations as required; mileage may vary and may exceed miles per day depending on service area and home location
- Perform other reasonable duties as assigned

**Qualifications & Experience**

**Required:**

- Graduate of an accredited NP or PA program
- Active, unrestricted license in California
- National board certification (AANP, ANCC, or NCCPA)
- Minimum 1 year of clinical experience (wound care, geriatrics, internal medicine, or similar)
- BLS certification
- Valid California driver's license and reliable transportation

**Preferred:**

- Wound care certification (CWSP, CWS) or willingness to obtain within 6 months
- DEA registration not required but preferred
- Experience in skilled nursing, home health, or mobile care settings
- Proficiency in EMR and digital documentation systems

- Active CA NP license
