**Job Description:**

- Schedule: Monday through Friday 8 a.m. to 4:30 p.m., with rotating on-call/weekends/holidays.
- Role Purpose: Evaluate clients and provide services requiring substantial and specialized skills. Implement preventive and rehabilitative nursing procedures, and instruct clients in self-care techniques. Coordinate all aspects of care for assigned patients when serving as case manager.

**Responsibilities:**

- Assess patient preferences and barriers to involvement in care, considering values and cultural needs.
- Develop, implement, and document individualized care plans with defined goals in collaboration with the interprofessional team.
- Promote respect, equity, and empathy in interactions with diverse populations.
- Evaluate changes in the patient's condition and update the plan of care as needed.
- Ensure nursing care is evidence-based for quality patient outcomes.

**Requirements:**

- Education: Nursing Diploma/Associate's in Nursing
- Licenses & Certifications: Valid Driver's License, RN

**Preferred Qualifications:**

- Education: Bachelor’s Degree in Nursing/Home Health
- Experience: 2-5 years
