**Job Description:**

- Under the supervision of a Registered Nurse (RN), provide routine care, observe patients' health, assist doctors and registered nurses, and communicate instructions to patients.
- Responsible for supervising and delegating tasks to nursing assistants or aides.
- Evaluate and report any changes in residents' physical or mental condition to appropriate personnel.
- Maintain residents' medical records in a confidential manner.

**Responsibilities & Duties:**

- Review, transcribe, communicate, and implement physician orders, obtaining RN signature as required by state regulations.
- Document care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines.
- Participate in medication management to promote optimal safety and effectiveness for residents by administering medications as ordered.
- Complete clinical admission paperwork for new admissions.

**Qualifications & Skills:**

- Graduate of accredited LPN School of Nursing.
- Current valid LPN license in the state employed.
- Valid CPR/BLS certification.
