Summary This is a Collaborative Care position and is open to LLMSW's $5,000 Sign-On Bonus Available! This position will be cover West Branch and Gladwin As part of the primary care treatment team, the Behavioral Health Therapist (BHT) identifies, assesses, triages , documents and manages primary care patients with medical and behavioral health problems. In addition, this BHT will utilize skill training through psycho-education and patient education strategies to develop patient self-management, population health programs, and will develop specific behavioral change plans for patients using best practice and brief evidence based treatment interventions. The BHT is a core member of the collaborative team, including the patient's medical provider and the larger primary care team. The BHT is responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload, while also providing crisis intervention and short-term consultation and support to the primary care team for patients in urgent need of same-day contact. Responsibilities (30%) Assesses the clinical status of patients referred by the primary care provider. Assists in the detection of "at risk" patients and intervenes to attenuate further psychological or physical deterioration. **Job Description:**

- Responsible for patient assessment and care under direct supervision of the provider
- Maintain appropriate documentation and coordinate care to ensure high-quality, compassionate patient care
- Oversee patients at office locations and act as liaison with other organizations and departments
- Order medical, pharmacy, and office supplies as needed
- Ensure compliance with policies/procedures of MyMichigan Health, OSHA guidelines, CLIA regulations, and other regulatory agencies
- Coordinate with and train new employees, students, and float staff
- Provide patient education, assist with specialty care, and manage symptom compliance for high-risk patients
- Perform general office duties as needed

**Responsibilities:**

- Triages all calls, schedules daily urgent/emergent add-ons, and relays test results
- Greets patients and performs routine pre-examination procedures, including vital signs and data recording
- Assists provider with examinations and procedures
- Prepares and cleans patient areas and equipment after procedures
- Performs patient care measures such as injections, dressing changes, and obtaining blood specimens
- Maintains a clean and safe environment for patients and co-workers
- May travel to satellite clinics and assist with procedures such as EKG, PFT, ABI, and NST
- Understands necessary computer functions and demonstrates critical thinking for patient management
- Maintains confidentiality and ensures patient rights in compliance with HIPAA
- Orders supplies and medications and maintains accurate documentation in medical records
- Participates in annual quality reviews and provides age-specific care as needed
- Performs other related duties as assigned
