**Licensed Practical Nurse (LPN)**

**Job Responsibilities:**

- Deliver high quality post-acute care in accordance with resident care plans, nursing best practices, facility policies, and all legal requirements
- Coordinate care with the wider nursing and clinical team
- Assist RNs and clinicians with tests and medical procedures
- Measure and record residents' vital signs and their physical and mental conditions
- Administer medications and vaccinations as directed
- Obtain resident medical histories and ensure that medical records are current, complete, and confidential
- Listen to residents and communicate their concerns to staff
- Engage with residents and family members to foster trust, relationship building, comfortable living conditions, and resident satisfaction
- Assist with the training of nursing and non-nursing support staff
- Promote a safe and inclusive environment for residents and fellow staff members
- Additional duties and responsibilities as assigned by leadership team

**Requirements:**

- Active and unencumbered Ohio LPN license
- Graduation from an accredited nursing program
- Self-starter with the ability to work independently and efficiently
- Must have reliable transportation
