Summary Eligible for a $2,500 Sign On Bonus for a two year commitment! Under direct supervision of the provider, will be responsible for patient assessment and care, and maintaining appropriate documentation. - Identify, assess, triage, document, and manage primary care patients with medical and behavioral health problems.
- Utilize skill training through psycho-education and patient education strategies.
- Develop patient self-management, population health programs, and specific behavioral change plans.
- Support and coordinate the mental and physical health care of patients on an assigned caseload.
- Provide crisis intervention and short-term consultation and support to the primary care team.

**Responsibilities:**

- Assess clinical status of patients referred by the primary care provider.
- Assist in recognizing and treating mental disorders and psychological problems.
- Provide brief behavioral interventions using evidence-based techniques.
- Use brief assessment tools and monitor patients for changes in clinical symptoms.
- Assist in program development and obtain patient education materials.
- Track patient follow-up and clinical outcomes using a registry.

**Other Duties:**

- Facilitate referrals for services outside of the health system.
- Assist in obtaining financial and other resources for patients and families.
- Conduct family meetings and integrate relevant theories into casework.
- Participate in scheduled caseload consultations and communicate recommendations.
- Utilize evidence-based scales to screen patients and guide treatment.
- Maintain documentation in all required systems.
- Participate in practice provider meetings and provide education/training.

**Requirements:**

- Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
- Master's degree in psychology or social work from an accredited college or university.
- Knowledge of psychology or social work as normally acquired through completion of a MA or Ph.D./Psy.D.
- Adherence to the American Psychological Association or National Association of Social Workers' Code of Ethics.

**Experience, Training, and Skills:**

- Membership in at least one professional organization preferred.
- Training and experience in casework principles and techniques in therapy.
- Knowledge of Integrated Behavioral Health practices, processes, and procedures.
- Interpersonal skills to communicate successfully with individuals and groups.

- Exposure to stressful situations, including trauma, grief, and death.
- Ability to wear personal protective equipment.
- Must be able to move freely about the facility and perform job functions.
- Vision and hearing necessary with or without an assisted device.
- Frequently required to sit/stand/walk for long periods.
- Some exposure to bloodborne pathogens and other potentially infectious material.
