The Team Manager leads the Patient Care Team, supervising, evaluating, and coordinating interdisciplinary members to ensure continuity of care. **Job Description:**

- Supervise, evaluate, and coordinate the interdisciplinary team members
- Assure continuity of care from admission to discharge or transfer to bereavement
- Serve as a patient advocate and coordinator for community social service and health care providers involved in patient care
- Maintain patient records from intake through discharge or transfer to bereavement
