    Job Description

    • Responsible for in-field oversight of clinicians who provide care and treatment to patients in their place of residence.
    • Handle incoming calls timely and follow up as necessary to ensure appropriate actions are taken to meet internal and external customer needs.
    • Coordinate training and educational programs for nurses within the field to improve clinical outcomes and competencies.
    • Assess staff learning needs and formulate specific plans and goals for the next evaluation period with employees.
    • Perform administrative call duties and serve as a role model and resource for staff.
    • Function as the first point of contact for field clinicians to assist in patient care escalations.
    • Coordinate at least bi-weekly case conferences with the clinical team to discuss patients' progress and revise care plans as needed.
    • Review and evaluate each case by reviewing services provided by clinicians, conducting conferences, record reviews, discussing and verifying impressions, instructing and guiding clinicians to promote effective performance and delivery of quality home care services.
    • Review patients' clinical diagnosis, medications, procedures, and clinical course.
    • Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing a plan of care.

    Requirements

    • Two years' management or supervisory experience within the past five years in a healthcare setting, preferably in home health.
    • Graduate of an accredited school of nursing; Bachelor's Degree preferred.
    • Current CA Registered Nurse License (RN).
    • Current Basic Life Support (BLS) Card.
    • Valid CA Driver's License with an acceptable driving record. All licenses and certifications must be current at the time of hire and sustained throughout your employment.

    Benefits

    • Wage range for Clinical Supervisor: $143,000.00 plus a generous annual bonus of $40,000.00, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.
