This role is responsible for providing primary care and diagnostic services, working collaboratively with the team to assist our patient population in chronic disease management and overall health promotion through individual intervention and ongoing education. This will be a newly built clinic and the physician will work with a team of a Nurse Practitioner, Mental Health Professional, Physical Therapist, and Medical Assistants.
