**Position Summary**

Provides care and services for children and families with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and client's living environment. Services may include comprehensive assessment and treatment planning, individual and family therapeutic interventions, and close collaboration with the clinical team to determine and guide service provision. Position will lead a team to navigate and meet the comprehensive needs of the child and family.

- Provide regular and consistent services and supports to clients identified as appropriate for community supports

POSITION SUMMARY As an Adult Therapist at Polara Health, you will work collaboratively with a diverse team of mental health professionals to provide individual and group therapy services to adult clients.
