They are actively involved in patient care assessment, planning, implementation, and evaluation with a specific focus on the use of MOUD for substance use disorder-related healthcare. Additionally, they collaborate with community partners for strategizing care plans for patients with substance use disorder needs. They actively participate in discharge planning by addressing health requirements within other facilities and upon discharge into the community. Effective collaboration and communication with various disciplines across the community are vital aspects of this role. QUALIFICATIONS : Registered Nurse with current RN license in State of Kentucky. CPR certification required. Two years nursing experience with demonstrated ability to inspire others to excellence. At least one year of chemical dependency experience is desired. Additional nursing experience in behavioral health, extended healthcare and acute healthcare preferred. ESSENTIAL FUNCTIONS: Treatment Planning: Collaborate with MAT/MOUD providers to develop comprehensive treatment plans for patients in residential treatment.
