Requirements

• Medical degree from an accredited medical school
• BC or BE in Family Medicine
• Completion of an accredited family medicine residency program
• Completion of a Sports Medicine fellowship required (for those that intend to practice Sports Medicine and Family Medicine)
• For specialty physicians - completion of an accredited fellowship program specific to medical specialty
• Valid state medical license to practice in North Carolina
• Current DEA
