Job Description

• Conduct comprehensive assessments to determine the care needs of patients.
• Develop, implement, and evaluate individual patient care plans in coordination with caregivers and physicians.
• Provide intermittent skilled nursing care to pediatric and adult patients at their residences.
• Supervise paraprofessional staff and licensed practical nurses, ensuring compliance with Colorado State law.
• Maintain accurate documentation and process orders, admissions, transfers, and discharge paperwork.
• Ensure ongoing communication with all disciplines involved in patient care.
• Adhere to federal, state, and agency policies and procedures.

Requirements

• Graduate from an accredited school of nursing with an associate or bachelor's degree in nursing.
• Active RN license in the state of Colorado.
• At least one year of nursing experience in home health care, with pediatric experience preferred.
• Knowledge of state and federal home health agency regulations and compliance standards.
• Current CPR certification from an American Heart approved course.
• Valid driver's license, reliable transportation, and active auto insurance.
• Ability to perform physical assessments and provide care in non-wheelchair accessible homes.
• Successful completion of a background check.
