Licensed Practical Nurse

Job Description

• Provide direct patient care under the supervision of an RN and within the scope of LPN licensure.
• Administer medications, treatments, and wound care following physician orders.
• Monitor vital signs, patient progress, and report changes to the nursing team.
• Ensure compliance with infection control protocols, HIPAA regulations, and state/federal healthcare guidelines.
• Assist with activities of daily living (ADLs) and provide compassionate resident care.
• Document patient care in electronic medical records (EHR/EMR systems).
• Collaborate with nurses, physicians, CNAs, and therapists to ensure optimal resident outcomes.

Requirements

• Active LPN License in the state (Required).
• Experience in long-term care (LTC), skilled nursing (SNF), assisted living, or rehabilitation preferred.
• Strong knowledge of medication administration, wound care, and infection control.
• Excellent communication, teamwork, and patient care skills.
• Ability to multitask in a fast-paced healthcare setting.
