Job Description

• Responsible for providing specialized nursing services to patients and caregivers at home or in alternative settings.
• Provides technical nursing and skilled interventions as ordered by the physician.
• Instructs, motivates, and assists patients/caregivers in learning and improving their ability to live independently at home.
• Ensures physician orders are followed and included in the patient's plan of care.
• Development and revision of the plan of care based on physician orders.
• Ensures nursing orders are carried out in a professional manner satisfying professional ethics, patient rights, and State and Federal codes and regulations.
• Continually monitoring patient outcomes and reporting changes to supervisors to prevent hospital readmission.
• Participates in effective communication amongst other clinicians providing care to the patient.
• Completes and submits all required documentation within established agency guidelines.
• Communicates the patient's progress to supervisor and makes changes as necessary under the physician's direction.
• Covers after hours, weekend, and holiday on-call to meet the branch's needs.
• Maintains confidentiality of all company and patient-related information. Protects patient rights as defined in the patient Bill of Rights.
• Attends and participates in staff meetings, required in-services, and other meetings as requested.
• Instructs patients/caregivers of the safety measures appropriate to care and environment and observes/implements standard precautions and infection control for all patients.
• Ensures accurate patient assessments are completed timely.
• Coordination of related services for patients.
• Responds to emergency patient needs timely and escalates to supervisors and physicians as needed.
• Always represents self in a professional and ethical manner.
• Takes professional responsibility for self-protection during required work in potentially hazardous situations.
• Completes all required patient care documentation and reports accurately and in a timely manner.
• Communicates with clinical staff, physicians, and other health care providers to assure the quality and continuity of patient care.
• Other duties as assigned.

Requirements

• Associate degree in nursing or equivalent.
• Current and unrestricted LPN clinical license.
• Two (2) years of clinical nursing experience is required.
• Five (5) years of related HME experience is preferred.
• Valid and unrestricted driver's license.
