Physical Therapy (PT) Case Manager
Location: Rochester, Minnesota
Job Description:
- The Physical Therapy (PT) Case Manager provides and oversees home health services for clients within the community.
- Collaborates with the client, their family, and the health care team to develop and implement goal-centered care plans.
- Provides skilled care to clients and clinical oversight to professional and unlicensed personnel.
- Builds and maintains respectful relationships with others.
- Reflects the company's mission and values in professional relationships.
- Performs initial and periodic assessment of clients to determine care needs.
- Collaborates with physicians and the home health team to update the client's care plan.
- Serves as a client advocate ensuring safety needs are addressed.
- Meets documentation standards ensuring accuracy and compliance.
- Provides education to clients, family, and caregivers.
- Provides direct care in accordance with the company's policies and the Minnesota Physical Therapy Practice Act.
- Coordinates client care with family, caregivers, and other health care members.
- Provides clinical oversight, delegation, and leadership to team members.
- Ensures effective financial management of the client's home health services.
- Manages work schedule effectively and adapts to changes.
Schedule:
- Monday-Friday 8am-5pm

Physical/Environmental Demands:
- Client care provided in individual client homes.
- May require transfers and lifting with adaptive equipment.
- Must be adaptable to various environments and community settings.
- Must be able to drive or use public transportation in all weather conditions.
- Heavy physical demand: moving weight between 10 to 100 pounds.
Qualifications:
- Currently licensed as a Physical Therapist (PT) in the State of Minnesota.
- Proof of negative TB testing.
- Access to a dependable automobile for travel.
- Valid driver's license and proof of current car insurance.
- U.S. Citizenship or evidence of valid Alien Work Permit.
- Disclose any conviction and criminal history records.
- Pass initial and ongoing background studies and screenings.
Skills and Experience:
- At least two years of experience in a health care facility or home care field.
- Demonstrates critical thinking skills and ability to follow procedures.
- Ability to work independently, adapt to change, and manage assignments.
- Excellent written and oral communication skills.
- Basic computer skills; experience with Electronic Medical Records preferred.
- Current CPR certification highly recommended.
