    Location: Hutchinson, Minnesota

    Job Description:

    • Provides direct care and oversees home health services for clients.
    • Collaborates with clients, families, and health care teams to develop goal-centered care plans.
    • Provides skilled nursing care and clinical oversight to professional and unlicensed personnel.
    • Reports to the Clinical Supervisor.

    Schedule:

    • Manage your schedule Monday-Friday.
    • Full-time with an on-call rotation every 5th weekend and 1 weeknight every other week.

    • Belle Plaine, MN
    • Jordan, MN

    Major Responsibilities:

    • Reflects the company's mission and values in professional relationships.
    • Performs assessments of clients, implements interventions, and evaluates care effectiveness.
    • Collaborates with physicians and the home health team on care plans.
    • Serves as a client advocate for safety needs and abuse prevention.
    • Ensures compliance with documentation standards and payer requirements.
    • Provides education to clients, families, and caregivers.
    • Provides direct care according to company policies and Minnesota Nurse Practice Act.
    • Coordinates client care with families, caregivers, and health care members.
    • Provides clinical oversight and leadership to licensed and unlicensed personnel.
    • Ensures financial management of client's home health services.
    • Manages work schedule and participates in the on-call process.

    Physical/Environmental Demands:

    • Client care provided in individual homes, requiring transfers and lifting with adaptive equipment.
    • Adaptable to various environments and community settings.
    • Must be able to drive or use public transportation in all types of weather.
    • Heavy lifting: 50 to 100 pounds occasionally; 25 to 50 pounds frequently; 10 to 20 pounds constantly.

    Qualifications:

    • Currently licensed as a Registered Nurse (RN) in the State of Minnesota.
    • Proof of negative TB testing.
    • Access to a dependable automobile for travel to company business stops.
    • Valid driver's license and current car insurance.
    • U.S. Citizenship or valid Alien Work Permit.
    • Disclose any conviction and criminal history records related to health services.
    • Pass initial and ongoing background studies and screenings.

    Skills and Experience:

    • At least two (2) years' experience in a health care facility or home care field.
    • Demonstrates critical thinking skills and clinical judgment.
    • Ability to work independently, adapt to change, and manage multiple assignments.
    • Strong professional relationship-building skills.
    • Excellent communication and teaching skills.
    • Basic computer skills; experience with Electronic Medical Records preferred.
    • Current CPR certification highly recommended.
