Physical Therapist (PT) - Home Health

Job Description

• Provide physical therapy services to patients in their homes
• Work 8 hours per day from 9:00 AM to 5:30 PM
• Possible weekend shifts as required
• Rate of $2,224 for 40 hours

Requirements

• 1+ years of recent home care experience
• Adult and geriatric experience
• Experience with Hoyer Lift and Tug
• Medication reconciliation skills
• OASIS experience
• Homecare Home Base EMR highly preferred
• Previous travel experience highly preferred
• New Jersey license
• BLS (AHA) certification
