Job Description

• Assist with activities of daily living (ADLs) and monitor resident health status
• Administer medications and treatments
• Document resident care and report all changes
• Follow physician orders and established care plans
• Promote a safe, clean, and supportive environment
• Provide emotional support and companionship

Requirements

• Must hold an active LPN/LVN license in the practicing state
• LTC/SNF/Assisted Living experience preferred
• Strong clinical, communication, and care skills
• Able to follow care plans and collaborate with staff
