    Geriatric/Internal/Family Medicine

    Join our team in Wilmington, DE, where you will provide essential care to geriatric patients in both inpatient and outpatient settings. This position is vital for ensuring that seniors can maintain their independence and quality of life.

    Job Description

    • Be part of or join the Program of All-Inclusive Care for the Elderly (PACE) organization.
    • Provide nursing home care.
    • Make patients able to live safely in the community at the time of enrollment and live in a PACE service area.
    • Provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.
    • Carry out medical procedures as indicated.
    • At the direction of the Medical Director, review medical outcomes/statistical and utilization data, including reviewing the necessity of hospitalizations and use of specialists.

    Requirements

    • Active DE License
    • Board Certified in Geriatric Medicine (Accept Internal Medicine/Family Medicine)
    • BLS certification
    • DEA registration
    • CDS/CSR certification
    • At least 1 year of experience working with frail or elderly population
    • EMR TruChart experience (preferred)
