Are you interested in a PRN (as needed) opportunity as a Physical Therapist? Adoration Home Health Care is seeking an experienced, compassionate, and committed PT to join our team in Tennessee! Job Description:

• Provide safe patient care and deliver individualized physician care plans to ensure optimal care experience
• Work as part of an interdisciplinary team, supporting both patients and their families
• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Requirements:

• Minimum Bachelor's Degree in Physical Therapy from a college or university with an accredited Physical Therapy program
• Current, unrestricted license as a Physical Therapist in state of operation
• Current BLS or CPR in accordance with state regulations
• Minimum one (1) year of PT experience preferred
• Valid driver's license, reliable vehicle, and proof of car insurance

Community Overview:

Coverage area includes Madison, Hardeman, Chester, McNairy, Henderson, Hardin, Decatur, and Somerville. This position offers a PRN schedule with 8-10 hours per week during day shifts, providing the flexibility needed to balance professional and personal commitments.
