Private Duty LPN
Join our team as a Private Duty LPN, providing personalized one-on-one in-home care in Humboldt, TN. This role is ideal for a dedicated nurse looking to work in a supportive environment while delivering critical care for a patient requiring trach, vent, and G-tube support.
Job Description
- Perform selected nursing procedures within the scope of LPN practice
- Provide patient and family teaching according to the plan outlined by the RN
- Monitor patient's vital signs and make specific measurements and observations
- Monitor and record intake and output. Report significant changes immediately
- Observe, assess, and record patient's physical and mental condition and report any changes
- Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly
- Attend and participate in multi-disciplinary patient case conferences, staff meetings, and in-service education programs as assigned
Requirements
- Must be trach/vent certified (training available)
- Experience with G-tube care
- Current LPN license in Tennessee
- Valid driver's license and reliable transportation
- Minimum of one year of nursing experience due to training required for ventilator certification
- Licensed Practical Nurse / LPN license in good standing
- Previous private duty, home health, hospice, hospital, or skilled nursing experience preferred
- Active CPR certification
- Familiarity with electronic health records
- Strong problem-solving and critical thinking skills
- Reliable transportation, valid driver's license, and current automobile insurance
