Private Duty LPN - Greenfield

Adoration Home Health is looking for a compassionate and skilled LPN to join our private duty team in Greenfield, TN. This overnight position focuses on one-on-one care, with specific needs for G-tube management and seizure monitoring in a supportive home environment.

Job Description

• Provide specialized nursing care with G-tube management and seizure monitoring.
• Record and track patient vital signs, intake, and output through the night.
• Administer medications as prescribed, providing patient and family education as needed.
• Collaborate with the healthcare team to ensure a high level of continuity and quality in patient care.
• Perform selected nursing procedures within the scope of LPN practice.
• Provide patient and family teaching according to the plan outlined by the RN.
• Monitor patient's vital signs and make specific measurements and observations.
• Monitor and record intake and output. Report significant changes immediately.
• Observe, assess, and record patient's physical and mental condition and report any changes.
• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly.
• Attend and participate in multi-disciplinary patient case conferences, staff meetings, and in-service education programs as assigned.

Requirements

• Active LPN license in Tennessee.
• Previous private duty, home health, hospice, hospital, or skilled nursing experience preferred.
• Experience with G-tube and seizure care is preferred but not required.
• Active CPR certification required.
• Familiarity with electronic health records.
• Strong problem-solving and critical thinking skills.
• Reliable transportation, valid driver's license, and current automobile insurance.
