Registered Nurse - Home Health

Job Description

• You will make a difference in our patient's lives by providing safe patient care.
• Deliver individualized nursing care plans to ensure our patients have an optimal care experience while working as part of an interdisciplinary team, supporting both patients and their families.
• Implement the treatment and rehabilitative strategies outlined in the patient care plan, including medication management, rehabilitation treatments, coordination of medical needs, and budget monitoring.
• Maintain thorough and timely documentation of care provided to ensure accurate patient record and billing.
• Conduct assessments and approve plan of care and service delivery methods.
• Follow consumer health plans, provide and promote consumer choice, and facilitate consumer independence.
• Perform competency and skills evaluations on employees who provide direct care.
• Other tasks as assigned.

Requirements

• Must be at least 21 years of age.
• Active Registered Nurse (RN) license in good standing in state of operation.
• Bachelor's Degree in nursing or related health and human services field from an accredited college or university OR an Associates Degree in nursing from an accredited two-year program.
• Valid driver's license, reliable vehicle, and proof of car insurance.
