Job Description

• Meet with patients in their place of residence for physician-ordered care including wound care, medication management and administration, and education on the patient's diagnoses.
• Use knowledge and critical thinking to communicate with physicians, clinical supervisors, and healthcare team members appropriately about changes in the patient's condition to get the plan of care quickly and efficiently updated.
• Ensure that the one-on-one setting allows for quick connections with patients to guarantee success in their recovery back to independence and make a positive lasting impression.

Requirements

• Currently licensed as a Licensed Practical Nurse (LPN) in the state of Idaho or other compact state.
• Two (2) years of nursing experience in community/home health or medical/surgical experience is preferred.
• Obtain and maintain current BLS CPR certification.
• Licensed driver with an automobile that is in good working order and ability to drive in all weather conditions to ensure reliability for patient care.
