Job Description

• Pay per visit
• Sup/Post Hospital: $60
• Reassessment: $90
• Initial Admission: $120 weekday / $180 weekend
• Visiting Client Territory: Corning
• Schedule: Visits planned with the required presence of the patient's aide
• Office Address: 4 East William Street, Corning, NY 14830

You will:

• Perform on-site supervisory and teaching visits to evaluate caregivers
• Provide clinical orientation to caregivers on assigned home care cases
• Participate in the training of new nurses and in-services for field staff
• Perform initial nursing assessments
• Counsel patient/caregiver in meeting nursing and related needs
• Assign and coordinate services provided
• Assist physician in developing a plan of treatment/care and make necessary revisions based upon patient's condition/needs
• Prepare clinical progress notes ensuring any changes in the patient's condition are reported
• Monitor clinical records for complete and accurate documentation

Requirements

• Registered Nurse (RN)
• CPR Certified
• 1 year of Home Care experience required
• 1 year of supervisory or management experience required
• Valid NYS Driver's License, insurance, registration, and reliable car
