Job Description
- Care and compassion are at the heart of what you'll do as an Occupational Therapist (OT). You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population.
- You'll provide home-based therapy care for patients as directed by the attending physician.
- Interact with patients' families while caring for your patients and experience the rewarding privilege to be part of every step of their recovery journey.
- Provide comprehensive skilled services as ordered by the attending physician.
- Educate patients and their family members and ensure the safety of the patient.
- Accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.
Major Areas of Responsibility

Plan of Care: Provide skilled occupational therapy as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff. Develop an individualized OT plan of care and make adjustments as needs change.

Evaluation: Evaluate the patient's level of functioning by applying diagnostic and prognostic procedures.

Patient Outcomes: Use activities and home exercise programs to improve range of motion, muscle strength, and coordination through passive/active exercise and/or activity.

Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.

Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy. Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.

Supervision: Oversee and supervise total care of patient provided by COTA.

Communication/Collaboration: Communicate with the COTA on supervisory visit results.

Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

Operations: Attend in-service training and mandatory agency meetings.
Requirements
- Occupational Therapist with a current license in the state of employment.
- Minimum one-year experience as an OT in an acute care setting. Home care experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Capable of all physical demands.
