Job Description:

• Schedule: Monday-Friday 8:00a-5:00p. Wednesday 10a-7p.
• Job Setting: Outpatient
• Types of Cases: Primary and Chronic Care
• Credentialing Timeframe: 30-45 Days

Requirements:

• Must be Board Certified
• Must have an active Connecticut state license
• Coverage as soon as possible and ongoing
