Physical Therapist, Full Time
What you'll do: Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation, and/or instruction.
Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with the client, family, physician, other providers, and pay sources towards outcomes promoting client independence and well-being.
Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client's age, condition, and needs.
Implement client care plan activities based on treatment goals towards independent living for the client/family.
Requirements
- Bachelor's, Master's, and/or Doctorate Degree in Physical Therapy from an accredited institution.
- Current PT license in the State of Maine.
- Current CPR card required.
- 1-year Physical Therapy experience preferred. Home health experience preferred.
- A reliable car with insurance.
