    Job Description:

    • Provide direct, one-on-one medical care to infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
    • Conduct ongoing patient care and assessments.
    • Administer prescribed medication, treatments, and therapies.
    • Coordinate care and educate family members on patient clinical care to enhance positive outcomes.
    • Implement preventative initiatives to protect quality of care for patients.

    Requirements:

    • Active RN or LPN/LVN license (New Grads Welcome, training provided!)
    • Provide care in a client home setting.
    • Ability to make a positive and lasting impression.
