Physical Therapist (PT)

We are looking for a PT to do in-home visits in the Pueblo area! We service adult and geriatric clients on a per-visit basis. One year prior clinical experience as a licensed Physical Therapist is preferred. As a home care Physical Therapist, you will be an integral member of the team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home. This position will also travel to La Junta and Canon City areas, and a company car will be provided. Candidates will be required to pass the requirements to be eligible to drive a company car.

Job Description

• Evaluation; initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate times
• Developing and revising plan of care
• Coordination of services in consultation with home health staff
• Discharge evaluation and planning
• Documentation of all visits
• Supervision of therapy assistants as appropriate

Requirements

• Colorado Physical Therapy license
• One year of patient experience, preferred but not required
• Valid driver's license

Compensation

• $75,000 - $90,000 per year
