    Family Medicine Advanced Provider

    Job Description

    Responsibilities

    • Obtain patient history and perform assessments via observation, interview, and examination. Order, perform, and interpret diagnostic studies. Perform preventative health assessments, screenings, immunizations, and patient care.
    • Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
    • Counsel and educate patients and their families/caregivers concerning preventative health, treatment options, and community resources.
    • Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

    Requirements

    • BLS Provider obtained within 1 month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Verified by local credentialing office.
    • Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing is also accepted. Licensure is dependent on the state associate works in. Verified by local credentialing office.
    • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing or current home state license, if considered multi-state/Compact State, also accepted. Licensure is dependent on the state associate works in. Verified by local credentialing office.
    • Drug Enforcement Administration credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.

    Education

    • Master's degree in Nursing required.
