Job Description

• Coordinates care for patients with chronic conditions who require disease, medication, or resource management to ensure optimum utilization of resources, excellent service delivery, and positive quality outcomes.
• Assists with Employee Health and COVID-19 activities, such as the assessment, testing, and management of COVID-19 symptoms for staff and patients.

Requirements

• Current Wisconsin RN license or eligible
• CPR certification
• Ability to educate and provide counseling and emotional support with compassion and respect
• Ability to maintain patient confidentiality
• Previous clinic experience
• Prior Epic experience preferred
