    Wellness Nurse

    We are looking for a Wellness Nurse to join our leadership team at Autumn Village!

    Job Description

    • Complete initial and ongoing assessments
    • Handle admission of residents
    • Medication monitoring
    • Health monitoring/screening
    • Monitor diabetic blood sugars
    • Diabetic cares as needed, i.e., fingernail and toenail care
    • Checking lab test report sheets from the clinic, follow through with medication changes as needed
    • Complete and review progress notes
    • Develop and update care plans
    • Arrange doctor appointments and setting up transportation
    • Complete all nursing procedures
    • Audit charts
    • Schedule and supervise all staff in absence of director
    • Monitor TB tests for residents and staff
    • Be prepared to work the floor if needed
    • Use the appropriate lift per residents' care plans
    • Communicate to Director what follow-up will be needed over the week
    • Perform additional duties as assigned
    • On-call responsibilities

    Requirements

    • Valid Wisconsin Registered Nurse license
    • Three to five years nursing experience with a minimum of 2 years in long-term care
    • Must possess leadership qualities and excellent communication skills
    • Supervisory experience preferred
    • BSN preferred
    • Follow/adhere to organization, federal/state regulations and guidelines, including resident/client rights, standard precautions, fire safety, and first aid

    Physical Requirements

    • Lift/carry up to 25 pounds occasionally
    • Push/pull medication carts up to 300 pounds occasionally
    • Twist, stoop, bend, squat, kneel, reach, walk, and sit frequently
    • Able to communicate orally and/or in writing with residents, co-workers, families, and the public
