- Conduct and write diagnostic assessments to assess the nature of the problem and establish medical necessity.
- Develop and implement Individual Service Plans based on diagnostic assessments.
- Provide psychotherapy/counseling services through individual, group, and/or family methods.
- Review and update assessments, ISPs, and diagnoses to ensure proper services.
- Provide Community Support Program Services as defined in OAC 5122-29-17.
- Achieve and maintain established productivity standards.
- Provide crisis intervention, pre-hospital screening, and/or emergency services as needed.
- Make referrals to other services within the community and the agency.
- Attend and participate in staff meetings, training, and supervision.
- Consult and communicate client-related issues internally and externally for coordination of care.
- Serve other departments, programs, or service locations as needed.
- Complete all documentation timely per agency policies and requirements.
- Maintain effective working relationships with co-workers.
- Communicate program functioning and client care concerns to supervisors.
- Maintain current knowledge of applicable agency policies and procedures.

**Qualifications:**

- Capacity for assessing, collecting, organizing, and reporting data.
- Ability to develop and implement treatment plans.
- Skills in crisis intervention and emergency services.
- Knowledge of current applicable diagnostic standards.
- Effective verbal and written communication skills.
- Ability to establish clinically effective relationships with clients.
- Function as part of an organization by working with others and adhering to policies.
- Training in First Aid, CPR, and De-escalation techniques.
- Valid Ohio Driver's License, reliable transportation, and insurability.
- Ability to meet agency requirements for a criminal background check.

**Minimum Educational/Licensure Requirements:**

- Master's Degree in Social Work or Counseling from an accredited university.
- Licensed as an LPC/LPCC or (Masters Level) LSW/LISW, Counselor in Training (CT).

**Minimum Experience Requirements:**

- Knowledge of and adherence to federal, state, and local laws, standards of the Ohio Department of Mental Health and Addiction Services, and appropriate professional ethical standards.

**Physical Requirements:**

- Must be able to lift up to 25 lbs.
