Requirements:

• Board Certified - REQUIRED
• Active SC license - REQUIRED, no exceptions
• BLS - REQUIRED
• 100% CLEAN BACKGROUND - HIGHLY Preferred; may consider small malpractice from years ago depending on situation
• No new grads; must have great experience ratings and training
• Provider must be local - REQUIRED
