Requirements

• Board Certified - REQUIRED
• Active VA License - REQUIRED
• BLS/ACLS Certified - REQUIRED
• DOT Certification - REQUIRED
• Must be willing to cover locations as listed in job
• Availability must be noted at time of name clear - preference to providers that can cover all dates
• Immunization - Influenza - Current Flu Season; COVID; TB / QuantFERON TB Gold; MMR - required to note confirmation at time of name clear
