Clinical Social Worker

This position is responsible for addressing the psychosocial outpatient needs of patients and their families by providing clinical social work evaluation, individual, family, and group counseling, as well as cultural diversity and advocacy expertise linked to community resources. The role involves developing, implementing, coordinating, and evaluating client service plans in an outpatient environment, while collaborating with other healthcare providers and community service agencies to coordinate care and services. It is expected that the employee demonstrates behavior consistent with the core values while supporting the strategic plan, goals, and direction of the Performance Improvement Process at Blessing Hospital.

Job Description

• Provide clinical social work evaluation and counseling to patients and their families.
• Develop, implement, coordinate, and evaluate client service plans in an outpatient environment.
• Collaborate with healthcare providers and community service agencies to coordinate care and services.

Requirements

• LCSW or LCPC required.
• Excellent written and verbal skills.
• Adequate transportation is a must.
• 1-2 years of experience in individual/family therapy preferred.
• Independent clinical skills needed to provide psychotherapy sessions.
• Possess a current appropriate Illinois License.
• Valid driver's license and reliable transportation required.

Location and Schedule

• Location: Blessing Hospital Office Behavioral Health.
• Schedule: Full-Time, Days, Monday-Friday; 40 hours; 8 AM - 5 PM.
