    Admissions Hospice Nurse

    Capstone Hospice is currently in search of an Admissions Hospice Nurse to join our dedicated admissions team. This role will require the nurse to process and complete admissions to the Capstone Hospice program. The Admissions Nurse should have the expectation of completing an admission each day. The position requires driving to the patient's location to complete the admission within our service area. This position has the expectations of working Monday - Friday, 10:00 am - 7:00 pm; this is a salaried exempt position.

    Candidate must be a Registered Nurse (RN) that can practice in the state of Georgia. Candidate should have strong hospice knowledge to be successful in this role. This position is the first clinical impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature, knowledgeable in caring for the terminally ill, and excellent interpersonal skills are required to be successful in this role. The goal is to help patients live their final days with dignity and comfort.

    Job Description

    • Complete admissions per day when needed
    • Collaborate with physicians and other professionals to form an initial plan of care
    • Complete documentation within 12-24 hours of admission visit
    • Provide support and comfort according to individual patient's needs
    • Work with caregivers and patients to assess goals of care
    • Maintain accurate records

    Requirements

    • Graduate from an accredited school of nursing
    • Currently licensed as a registered nurse (RN) to practice in the state of Georgia
    • Minimum of 2 years as a registered nurse
    • Active patient contact within the past 2 years, preferred
    • 1-2 years hospice admissions knowledge or experience
    • Active CPR certificate
    • Must have current valid driver's license, auto liability insurance, and your own reliable transportation
    • Must be able to provide evidence of annual TB test and other state-required tests or examinations

    Position Factors

    Requires physical effort most of the day including walking, exposure to humidity changes. Exposure to patient homes, including their families and pets within the home. Exposure to different social, racial, cultural, and religious modes. Must possess sight/hearing senses or use of appropriate adaptive devices that will enable senses to function at a level to meet the essential duties of the position. Exposure to mental and emotional stress of the patients, caregivers, and families.
