Job Description

• Oversee all home-based skilled nursing care based on the plan of care
• Complete and submit Oasis Assessments, Transfers, Resumption of Care, Discharges, and Significant change in condition in accordance with Agency time frames for patients located within the Petersburg, Colonial Heights, Hopewell, Chesterfield, and surrounding areas

Requirements

• Graduate of an approved school of professional nursing
• Current and valid Virginia RN License
• Passionate and motivated to provide care within a patient's home
• Ability to exercise initiative and independent judgment
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Ability and willingness to travel within a given territory
• Willingness to work with multiple patients in varying home locations

Preferred Qualifications

• Two (2) years' home health nursing experience
• Previous experience with Home Care Home Base EMR system strongly preferred
• Wound Care experience or Certification strongly preferred
