Job Description:

• Visit patients within our Little Rock service area to provide care in their homes or assisted living facilities
• Develop and execute personalized physical therapy care plans for each patient
• Record patient progress, including admissions, discharges, OASIS assessments, and regular updates to the Director
• Complete any other assigned tasks

Requirements:

• Active Arkansas Physical Therapy license or registered in the state of Arkansas
• A minimum of three professional references on the application or upon initial contact
• Home Health experience is a plus but not required
