Job Description:

• Provide one-to-one nursing care in a home environment
• Work with self-sufficient autonomy, empowered to make a real difference in clients' lives
• Start the day by touching base with the care team and accessing client information via EHR platform
• Perform routine care such as trach and g-tube care, and document treatments in the Point of Care platform
• Build clinical expertise while shaping the lives of clients and their families

A Day in the Life:

• Begin by touching base with your care team, including the family or the nurse you're relieving
• Engage with clients, providing necessary care such as ventilator management, breakfast, and hygiene routines
• Participate in activities that enrich the client's day, like outdoor exploration or reading
• Document all care and medication details electronically

Requirements:

• Valid Pennsylvania LPN License or Multistate License
• PPD or Chest X-Ray
• Valid BLS CPR card obtained in person
• Valid driver's license
• Experience with G-tube, trach, vent, or willingness to train
