    Clinical Team Leader

    Join our dynamic team as a Clinical Team Leader (CTL) for the Lexington and Bedford locations. This position involves management of the Bedford office and staff, with the primary work location in Lexington and occasional travel to Bedford. Leadership on-call responsibilities are also part of this role.

    Job Description

    • Serve as a team leader, clinical resource person, role model, mentor, and facilitator for staff and department operations.
    • Lead staff, resolve routine issues, and create a positive, productive, and fiscally responsible team.
    • Remain aware of organizational developments and initiatives and implement as needed for the team.
    • Refer complex or potentially significant issues to manager/supervisor, including but not limited to issues regarding compensation, work hours, working conditions, hiring, and formal corrective action.
    • Initiate and facilitate effective open two-way communication and resolution of work site operational issues that support service area goals and objectives.
    • Maintain clinical competencies and provide professional care to patients of all ages in response to agency needs.
    • Assess, plan, implement, and evaluate patient care to achieve optimum patient outcomes for patients as appropriate to service area.
    • Utilize leadership principles in mobilizing the Carilion Home Care/Hospice team to achieve positive patient outcomes.
    • Follow up on matters to ensure they are successfully resolved.
    • Assist manager/supervisor in assuring staff competence and contribute to staff performance development.
    • Identify areas of improvement and assist with the coordination of workflow processes of the home care/hospice team.
    • Collaborate with manager/supervisor regarding fiscal management; monitor staff productivity overtime, mileage, and supply usage to ensure appropriate resource allocation and expense control.
    • Assist with other agency responsibilities including implementing and managing daily activities of the office, maintaining regulatory compliance, reviewing documentation, ICD-10 coding, monitoring face-to-face documents, re-certifications, participating in performance improvement, and other quality improvement initiatives.
    • Ensure Joint Commission, Medicare, and other third-party payer guidelines are implemented and followed.
    • Conduct individual case conferences and participate in interdisciplinary team meetings to facilitate effective two-way communications.
    • Listen to staff members' needs and concerns and resolve items brought to their attention; follow up with staff members on identified issues and concerns as needed.
    • Initiate corrective action policy by employee coaching when indicated.
    • Promote a positive work environment by holding staff accountable for their conduct while using effective interpersonal skills.
    • Demonstrate and encourage SBAR communication.
    • Perform other duties as assigned.

    Requirements

    • Education: Graduate of an accredited Registered Nurse program. Bachelor's degree preferred.
    • Experience: Five years of clinical experience required. Demonstrated strong leadership skills; supervisory experience preferred. Home Health/Hospice experience preferred.
    • Licensure, certification, and/or registration: Currently licensed as a Registered Nurse in the state of Virginia or approved compact state. Must have a valid driver's license, automobile insurance, and personal vehicle for making home visits.
    • Life Support: Must obtain/maintain AHA BLS-HCP.
    • Other Minimum Qualifications: Demonstrates leadership qualities at current level of practice, good interpersonal and communication skills, and teambuilding skills required. Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
