LPN/RN Hall Nurse
Job Description:
- Provides nursing care to residents assigned
- Participates in planning and implementing unit activities on the shift and unit as assigned by the DON and/or RN Supervisor
- Assures that department personnel, residents, visitors, etc., follow established policies and procedures, including safety rules and regulations
- Provides accurate and consistent information to personnel, residents, visitors, family members, etc., as necessary
- Ensures resident rights are followed by all department personnel
- Maintains confidentiality of resident care information
- Maintains a stock level of medications, medical supplies, equipment, etc., and requisitions as required
- Notifies the resident's attending/alternate physician, RN supervisor, as well as the resident's legal guardian representative, when the resident becomes critically ill or injured
- Works with the facility's consultants and implements recommended changes as directed by the DON
- In cases of death, assures proper preparation for pick up and may announce death as allowed within state rules
- Makes oral reports/recommendations for nursing POC to the supervising RN/DON
- Maintains unit area cleanliness and safety
- Notifies physician and RN supervisor of resident accidents/incidents and completes necessary reports
- Takes TPRs, blood pressures, etc., and charts as required
- Gives/receives nursing report upon start and end of shift
- Monitors residents throughout the assigned shift to assure prescribed treatment is properly administered
- Communicates changes in resident condition to RN as needed
- Prepares/administers medications as ordered
Requirements:
- Graduate of an approved LPN program
- Training in rehabilitative and restorative nursing practices is preferred
- Experience in a hospital, skilled nursing care facility, or related medical facility
- Current CPR Certification
- Must function independently within the scope of practice for LPN
- Must meet state requirements for relevant licensures or certifications with no disciplinary action

- Follows appropriate safety and hygiene measures
- Maintains confidentiality of protected health information
- Reports noncompliance with policies, procedures, or regulations
- Reports any allegations of abuse or mistreatment of residents
- Participates in life safety and emergency drills and trainings
- Reports work-related injuries and illnesses immediately
- Follows established infection control policies and procedures
- Completes all assigned training and skills competency
- Attends care-plan meetings of assigned residents
- Participates in QAPI or facility assessment activities as needed
