Integrated Behavioral Health Therapist
Job Description:
- Provides direct integrated co-occurring behavioral health and substance use services in support of integrated primary care and behavioral health services.
- Maintains a caseload of patients engaging in co-occurring behavioral health and substance use services to provide therapeutic support that meets their life needs.
Major Duties and Responsibilities:
- Conducts brief, evidence-based and developmentally appropriate screenings.
- Conducts time-limited assessments.
- Applies knowledge of evidence-based practice to individuals, couples, groups, and families, including adults, children, and adolescents.
- Assesses both behavioral health and substance use needs and determines the most appropriate clinical interventions.
- Provides emergency assessment of patients in crisis and facilitates psychiatric hospitalization if needed.
- Assesses discharge readiness throughout the course of treatment.
- Creates and implements integrated treatment plans, ensuring timely access to organizational and external services.
- Independently evaluates treatment progress and modifies plans as indicated.
- Provides referrals for internal and external services.
- Engages patients and/or families not engaged in services through outreach and follow-up.
- Provides consultation to medical providers and other service providers to facilitate a coordinated continuum of care.
- Participates in staff meetings and system change initiatives within Chase Brexton Health Care.
- Ensures progress notes are accessible to the patient's care team.
Compliance, Policy, and Procedure:
- Documents all behavioral health services as directed by CBHC policy, state requirements, and professional standards.
- Adheres to departmental productivity standards and policies.
- Assesses risk using standardized measures (e.g., CAMS; C-SSRS).
- Demonstrates appropriate use of billing orders for all clinical documentation.
- Utilizes Electronic Health Record to support and improve integrated healthcare.
Skills and Abilities:
- Ability to provide culturally relevant services and identify/address disparities in healthcare access and quality.
- Adapt services to cultural norms and promote diversity among providers in interprofessional teams.
- Familiarity with open access models in community mental health and healthcare settings.
- Adaptability to changes in the healthcare field both statewide and nationally.
Education and/or Experience:
- Required: Master's Degree in Social Work, Counseling, Psy D, or PhD.
- Licensure Required: Licensure in the state of Maryland.
- Licensure Preferred: LCSW-C, LCPC, Psychologist.
- Preferred: Two years of experience in outpatient mental health and/or substance use services with adults, adolescents, and/or children. Experience in a primary care setting is strongly preferred.
- Preferred: Multi-Lingual.
