High Fidelity Wraparound Facilitator

The HFW (High Fidelity Wraparound) Facilitator works as part of a Joint Planning Team to assist adolescents with emotional needs and their families in strengthening systems of natural support, improving the ability and confidence of family members in managing their own services, and organizing formal and community supports through a comprehensive, individualized, holistic support plan. The Facilitator will also work closely with the Juvenile Mental Health Treatment Court staff and the Mental Health treatment staff and supervisors to help coordinate care.

Job Description

• Implement the 10 Principles of High Fidelity Wraparound.
• Implement the 4 Phases of High Fidelity Wraparound.
• Ensure the Principles and Phases of the High Fidelity Wraparound process are delivered with the highest possible fidelity.
• Work with youth and family to create and maintain a team for each adolescent and family.
• Identify and link adolescents and families to natural and community resources.
• Help to coordinate all aspects of mental health care for the participating clients with the CGRC treatment programs, outside providers, and court-related professionals.
• Active coordination and collaboration with other CGRC supervisors to ensure that the clinical needs of the clients are being met.
• Attend and participate in MH planning and evaluation court meetings and client court meetings as needed.
• Manage a caseload of 8 - 15 families.
