Nurse Practitioner (NP)

Job Description:

• Responsible for the overall comprehensive care provided to the patients in the CHRISTUS SETX Health System.
• Coordinate care with the Medical Director utilizing training within the scope of practice.
• Conduct patient education and ensure accurate and timely documentation in accordance with regulating agencies and organizational policy.
• Participate in community events and marketing opportunities involving the CHRISTUS SETX Health System.
• Coordinate patient management and individualize a plan of care for each patient, working closely with the Medical Director and other team members.
• Take medical histories, perform physical exams, order and interpret laboratory studies, and provide treatment to patients with chronic diseases.
• Work closely with the RN and Medical Director to ensure follow-up calls are made.
• Assist in identifying additional visits or interventions that may be needed.
• Manage and adjust medications and educate patients.

Requirements:

• Current Licensure through the Texas Board of Nursing OR Licensed Physician Assistant by the Texas State Medical Board or the ability to obtain prior to employment.
• Valid DEA license or the ability to obtain one prior to employment.
• BLS obtained by the American Heart Association.
